Confidentiality

By accepting these terms, I consent that Sally Rusbridge Psychotherapy may release information to a specific individual or agency if it has been determined that a child, elder or any other person including myself is at risk or is currently being abused; If I as a client am in imminent danger to myself or others or if a subpoena of records has been requested.

I also understand that at anytime, Sally Rusbridge Psychotherapy may discuss aspects of my case with other colleagues keeping my full name and identity completely confidential unless I have given permission otherwise.

By booking a session with you hereby release Sally Rusbridge Psychotherapy from any liability or claims that could be made against her concerning your mental and or physical wellbeing during the work that has been outlined and agreed upon (now and in the future) by accepting the terms of use.

Scope of Practice

I understand that Sally Rusbridge Psychotherapy is NOT a Psychologist or medical practitioner of any kind and that hypnosis should not be considered a replacement for the advise and or services of a Psychiatrist, Psychologist or Medical Doctor. Sally Rusbridge Psychotherapy holds the right to refuse treatment, to cease treatment and refer me on at any time.

Participation

I give Sally Rusbridge Psychotherapy full permission to hypnotise me and to use a combination of modalities methods, knowing that by participating fully in the process and by listening to my personalised recording for 21 days, I play an important role in my overall success.

Guarantee

I understand that although attending a minimum of 4 consecutive weekly sessions with Sally Rusbridge Psychotherapy has a high success rate, Sandra cannot and does not guarantee results since my own personal success depends on many factors that Sally Rusbridge Psychotherapy has no control over, including my willingness and desire to affect the changes inside of myself.

Audio Recordings

I give Sally Rusbridge Psychotherapy full permission to make audio recordings that may include my voice. I understand that if a recording/s are made during or after my session/s, Sally Rusbridge Psychotherapy retains full copyright over any copies distributed to me.

Liability

You agree that, except for death and personal injury arising from our negligence, we shall not be liable in contract, tort, negligence, statutory duty or otherwise, for any loss or damage whatsoever arising directly or indirectly from or in any way connected with the site or your use of or reliance upon the content or any information you obtain by means of the site or your use of or reliance upon the content or any information you obtain by means of the site or any bookings or purchases you make of goods and/or services through your use of the site, including, without limitation, damage for loss of profits, loss of business, or any other consequential or pecuniary loss (even where we have been advised of the possibility of such loss or damage.)
